Athletics

In order to participate in athletics, complete and submit the following forms to the Athletic Training Office (Bldg. 2) PRIOR to participation:

• Athletic Physical Exam + Concussion Management Forms (valid for one calendar year)
• Student Participation and Parent/Legal Guardian Consent, Release, and Assumption of Risk Form (complete a new form for each participating sport)

Before competition begins, a mandatory parent meeting will be held by the Athletic Department. This meeting is scheduled for Fall, Winter, and Spring seasons. 

Check the athletic calendar for those dates.

Need a Sports Physical? Schedule an appointment with the Hawaii Keiki Nurse and complete consent form.
